Job Summary and Qualifications

We are currently looking for an ambitious Clinical Rehabilitation Liaison to join our on-site team. As our Clinical Rehab Liaison, you are responsible for coordination of the timely and accurate completion of the Patient Assessment Instrument for each rehabilitation inpatient. You will act as a liaison between internal and external referral sources and internal operations. Your duties and responsibilities may include:

  • Establish relationships with case managers, physicians, payers and other referral sources
  • Ensure appropriate patient placement through clinical screenings and assessments in collaboration with both hospital Case Management and the Rehabilitation Medical Director
  • Work closely with hospital Case Management to ensure a responsive and smooth referral flow and admission process for the acute-care, inpatient rehabilitation program

Requirements:

  • Graduate of an accredited school in a clinical specialty
  • Two years’ experience in inpatient rehabilitation setting, preferred.
  • Must be a Florida-licensed professional in a clinical healthcare specialty (RN or CCRN, Physical Therapist, Occupational Therapist, Speech Therapist, Respiratory Therapist)
  • Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, ability to follow verbal instructions, PC skills
